- Dr Curtis only books in-person appointments unless there are extenuating circumstances
- Please limit your appointments to one request per visit
- Dr Curtis typically works Wednesday through Friday

- ALL patient requests will require an appointment with your Primary Care Provider.
- Please do not ask your pharmacy to fax a request for prescription refills, again you will require an appointment for all medications.
- To book Physicals, Well Baby Checks, Mental Health or Procedures you must call the clinic.
- Please note that we require 24hr notice of appointment cancellations otherwise we will have to charge for the missed appointment.
