- To book a phone or in-person Standard Appointment with Dr Piek please call the office
- ALL patient requests will require an appointment with your Primary Care Provider.
- Please do not ask your pharmacy to fax a request for prescription refills, again you will require an appointment over the phone or in person for all medications.
- To book Physicals, Well Baby Checks, Mental Health or Procedures you must call the clinic.
- Please note that we require 24hr notice of appointment cancellations otherwise we will have to charge for the missed appointment.
Dr Kerry Baerg is retired as of Friday, November 21st and Dr Johannes Piek has now taken over his practice.
